This affords both patients and their care teams a comprehensive, 360-degree view of an individual's health status. Data can be entered manually, or flow in automatically through existing Electronic Medical Record (EMR) systems or automated workflows within the platform. The Trustr platform provides a comprehensive solution for remote patient monitoring through the use of AI Vitals, both for device-based and deviceless applications. Personalized thresholds can be established by the care team for each patient through the Physician module. When these thresholds are exceeded, the system will promptly alert the care team, enabling a digital triage to take place and any necessary interventions to be administered.
